Health First Nutritional Program

Thank you for your interest in the HealthFirst Nutrition and Weight Management Program offered by the Delaware Institute for Reproductive Medicine. We are eager to assist you with your nutrition goals and to provide you with an educational experience which will aid you in improving your overall health, lifestyle, and eating habits. The program focuses on identifying your individual metabolic rate, modifying caloric intake to facilitate weight loss, and beginning and/or maintaining a physical activity program.

Appointment Instructions

Client Paperwork - The 3 day food intake form and program registration must be received via fax at 302.738.3508, or emailed to: cabboud@ivf-success.com, or US mail at 4745 Ogletown-Stanton Road Suite, 111 Newark, DE 19713 on the Monday prior to your appointment. Failure to return the form will result in a rescheduling of your appointment.

Pre-Appointment Requirements

In order to gather the most accurate date from your assessment the protocol below must be followed prior to your appointment:

• Do not drink or eat anything other than water for at least 4 hours prior to your appointment.
• Do not exercise (aerobic or strength training) for at least 4 hours prior to your appointment.
• Do not smoke or use nicotine for at least 1 hour prior to your appointment (includes the use of smoking cessation products such as gum or patches).
• Do not consume caffeine products (such as coffee, soda, energy drinks), nutritional supplements or medications containing ephedra, ephedrine, Ma Huang, pseudoephedrine, etc. for at least 4 hours prior to your appointment.
• Remember to bring a snack or your lunch to your appointment.

Appointment Attire

You will not be performing exercise during your appointment, so casual or comfortable clothing is suitable.

Payment/Billing/Cancellation

A one-time, $50 program fee is required at the time of your initial appointment
