INSURANCE PLANS WE ACCEPT

Delaware Institute for Reproductive Medicine (DIRM) is a participating provider with Aetna, Blue Cross Blue Shield of Delaware (all products), all out-of-state Blue Cross Blue Shield PPO plans, Cigna and Coventry. Being a participating provider with your insurance carrier means that we have a contract with them and that we accept their allowable fee (not payment amount) as payment in full. We will file all of your claims for you and will assist you in appealing any claims that were denied incorrectly.
Not being a participating provider with your insurance carrier (all plans not noted above) means that we do not have a contract with them. As a courtesy, we will file claims with some (but not all) insurance carriers with which we have had a good relationship in the past. However, we will not accept their “usual and customary” allowable amount as payment in full for services rendered, even if their check to us states that by endorsing their check, we agree to accept the amount as payment of the claim in full. You will be responsible for any claims not paid after 30 days. You are responsible for 100% of DIRM’s fee. We cannot assist in the appeal of any denied claims.
You will be responsible for payment of any amounts not allowed by your plan. Payment in full is due immediately upon receipt of your statement.
Whether or not we are a participating provider with your insurance carrier, please recognize that not all services are a covered benefit in all contracts. Some plans have no infertility benefits, neither diagnostic nor treatment, other plans offer diagnostic coverage only, while other plans offer both diagnostic and treatment benefits. Some plans with treatment benefits cover only IUI (intrauterine insemination) but not IVF (in-vitro fertilization). Other plans cover both IUI and IVF. Most plans with treatment benefits usually have a lifetime dollar limit or lifetime cycle limit.
