FINANCIAL POLICY

Welcome to the Delaware Institute for Reproductive Medicine (DIRM), Delaware’s leader in the field of infertility since 1986. Our practice is committed to state of the art, cutting-edge technology, with exceptional pregnancy rates creating over 5500 babies in the Delaware valley. We understand the high cost of fertility treatments and constantly work to keep our prices competitive and to help you maximize your insurance benefits.
We have implemented the following policies to assist you in this journey which help us to keep costs under control. Full payment or co-payment is due at the time of service. All medications dispensed from our offices must be paid for in full upon receipt of the medications. We accept cash, checks, MasterCard, Visa, Discover and American Express.
It is our policy to obtain a social security number from all patients. By asking us to file an insurance claim for you and wait for payment, we are extending credit to you. If you do not want to provide us with a social security number, we require one of the following:
  1. A credit card number and expiration date to keep on file. All unpaid balances after insurance has paid will immediately be applied to your credit card. Your credit card information will be destroyed either when you provide us with a social security number or when your relationship with our practice ends.
  2. Payment in full is required at every visit and a receipt will be provided so that you may seek reimbursement from your insurance company.

NEW TREATMENT CYCLES

Patients who begin a new treatment cycle must have financial clearance from the Billing Office. Patients will be required to pay in full any outstanding patient/partner balances before a new treatment cycle can commence.

INSURANCE COVERAGE:

Before you begin infertility treatment at DIRM, we urge you to check with your insurance company as to your coverage. Many insurance companies provide online access to your benefits or call centers to assist you. If you still need help in determining whether or not your insurance policy covers infertility treatment, please contact us.

Questions to ask your insurance company:

  • Is an infertility diagnosis a covered benefit?
  • Do I have diagnostic coverage only or is treatment also covered?
  • If treatment is covered, is intrauterine insemination covered? If so, is there a lifetime dollar limit or a lifetime cycle limit?
  • Is in-vitro fertilization covered? If so, is there a lifetime dollar limit or a lifetime cycle limit? Is freezing of embryos covered?
  • Are injectable medications covered? If so, is there a dollar limit? Is there a preferred specialty pharmacy?
  • Is there a pre-existing condition clause? If so, what’s the waiting period?
  • Do I have to be married to use my treatment benefits? If so, for how long?
  • Do I have an annual deductible?
  • Do I have a copayment? If so, what is it?
  • Do I have 100% coverage or do I have a coinsurance?
  • Do services need to be precertified?
We must emphasize that as a medical care provider, DIRM’s relationship is with you and not your insurance company. Your insurance policy is a contract between you and your insurance carrier and we are not a party to this contract. While the filing of insurance claims is a courtesy that we extend to our patients, all charges are your responsibility. We realize that temporary financial issues do arise and we encourage you to contact us promptly for assistance in the management of your account.
